ENTER A CARE PLAN WITH CLINICIAN
The process begins in the clinician's office by entering the details of the individual care plan on a smartphone; for example, what medications to take, and what vital signs to measure.
If a friend or family member is helping the patient manage their care, the patient can decide which features they would like to share with them.
INPUT INFORMATION DAILY
Each day, enter the care plan information on the patient's phone: weight, blood pressure, blood sugar level; whatever was agreed upon as part of the care plan. Did the patient take their medication? Has the patient answered the clinician's questions about their symptoms and overall wellness? Do they need to send a message or question directly to their clinician's office?
At any time, the patient can view any of their data charted right on their own mobile device so they can view trends and monitor their adherence with the plan laid out with the clinician. At all times, data is checked against targets set by the clinician, resulting in immediate action and support for them if they are out-of-range.
